The Minnesota Public Health Impact Awards
The Minnesota Public Health Impact Awards recognize and celebrate people and organizations that have made meaningful contributions to public health in our state. These awards were formerly known as the Community Health Awards.
Award nominations are now open through Monday, Aug. 14.

There are several award categories available. Information about each award, including selection criteria, eligibility, and past recipients, can be found by clicking on the links below:
- Jim Parker Leadership Award: This award recognizes persons or groups who have demonstrated significant statewide leadership in public health.
- Jack Korlath Partnership Award: This award recognizes individuals whose work has strengthened the state-local partnership.
- Lou Fuller Award for Distinguished Service in Eliminating Health Disparities: This award recognizes individuals or groups who have made a significant contribution to the elimination of health disparities in Minnesota.
- Commissioner's Award for Distinguished Service in Community Health Services: This award recognizes individuals or groups who have contributed to advancement in the field of public health.
- Award for Outstanding Dedication to Local Public Health: This award recognizes individuals who have made a significant contribution to the promotion and implementation of local public health in Minnesota.
- Certificates of Recognition: Certificates recognize individuals or groups who have demonstrated a significant commitment to public health.
The nomination form includes questions related to each award's selection criteria. The selection committee will review and score nominations based on how well the responses demonstrate that the nominee meets the criteria.
About the Public Health Impact Awards
Public health professionals in Minnesota nominate their peers for these awards annually. Following the nomination period, a selection committee will review submissions and notify recipients. Awards will be presented at events later this fall.
