Eligibility
Individuals eligible for this award include current State Community Health Services Advisory Committee (SCHSAC) members and alternates, local and Tribal public health staff, Minnesota Department of Health staff, local advisory committee members, elected officials, and/or volunteers.
Groups eligible for this award include: Any combination of eligible individuals, teams, or groups from the community and teams or groups from partner organizations, whose work demonstrates a significant commitment to public health.
