HMOs and County Based Purchasers
List of Health Maintenance Organizations (HMOs) and County-Based Purchasers (CBPs)

HMOs

Allina Health and Aetna Health was licensed as an HMO in Minnesota in June 2022. They currently have no enrollment.

Founded in 1974, Blue Plus HMO is a subsidiary of Aware Integrated, Inc. Blue Plus offers a range of fully insured commercial health plans as well as Medicare plans. Blue Plus also contracts with the Minnesota Department of Human Services to provide public health plans including Medical Assistance and MinnesotaCare. Blue Plus offers plans statewide. 

Group Health Plan, Inc. was founded in 1957 as a staff model clinic. Group Health Plan, Inc. is associated with HealthPartners. In 2024, HealthPartners closed the Group Health Plan, Inc. HMO license.

HealthPartners, Inc. is an affiliate of HealthPartners Insurance Company. HealthPartners currently offers plans in the individual and group markets as well as the Medicare market. In addition, HealthPartners, Inc. contracts with the Minnesota Department of Human Services to provide services to those enrolled in public health plans including Medical Assistance and MinnesotaCare.

Hennepin Health was established by Hennepin County in 1983 and became licensed as a Minnesota HMO in 1995. Hennepin Health is the only HMO in Minnesota operated by a local unit of government. Hennepin Health enrollment is limited to those covered by Medicare and Minnesota Department of Human Services programs.

Humana Inc. was founded in 1961 as a nursing home company. It shifted its business focus to hospitals in 1972, and created its own insurance plan in the 1980’s. Humana of Wisconsin was certified to operate as a Health Maintenance Organization in Minnesota in April of 2019. Humana currently offers Medicare Advantage plans in Minnesota.  

Medica Health Plans was founded in 1975 under the name Physicians Health Plan of Minneapolis. It is an affiliate of Medica Insurance Company and Medica Community Health Plan. Medica HMO offers health plans in the Medicare market and contracts with the Department of Human Services to provide managed care plans to those in Medical Assistance and MinnesotaCare.

Medica Community Health Plan (MCHP) (formerly known as Medica Health Plans of Wisconsin) was initially licensed as an HMO in the State of Wisconsin in 1996. In 2013, MCHP became licensed as a Service Corporation in the State of Wisconsin and Minnesota as a MN Statute 62C Nonprofit Health Service Plan Corporation. In 2018 MCHP changed its license back to an HMO in the State of Wisconsin and withdrew its MN Statute 62C license in the State of Minnesota. Medica Community Health Plan was licensed as an HMO in Minnesota in 2020. MCHP currently has no enrollment in Minnesota.

Quartz Health Plan MN Corporation is an affiliate of Quartz Health Plan Corporation which was established in 1955 in Wisconsin as Gundersen Lutheran Health Plan. The Minnesota HMO was licensed in 2012 and offers individual and group plans in Fillmore, Houston, Olmsted and Winona counties in southeast Minnesota.

Sanford Health Plan (formerly Sioux Valley Health Plan) was established in 1998 as an affiliate of Sanford Health Plan of South Dakota. Sanford offers coverage in the group market.

UCare Minnesota was established in 1984 by the Department of Family Practice at the University of Minnesota Medical School. UCare offers individual market and Medicare plans as well as contracts with the Department of Human Services to provide care to those enrolled in public programs including Medical Assistance and MinnesotaCare. 

 

UnitedHealthcare is a for-profit company headquartered in Minnetonka, Minnesota. UnitedHealthcare was founded in 1977 to manage the newly created Physicians Health Plan of Minnesota, an early health maintenance organization. Today, it is the insurance arm of UnitedHealth Group, Inc. and has a national presence, providing health benefit plans and services for large employers, managed care solutions to state Medicaid programs, and Medicare services. In February 2019, UnitedHealthcare of Illinois, Inc. became the first for-profit organization certified to operate as a health-maintenance organization in Minnesota. UnitedHealthcare of Illinoise, Inc. currently offers plans in the individual, group and Medicare markets. 

CBPs

IMCare is administered by Itasca County Health & Human Services (ICHHS). IMCare contracts with the state (DHS) and the federal government (CMS) to provide services to people enrolled through one of the state's public health plans as well as those enrolled through Medicare. IMCare offers coverage in Itasca county.

PrimeWest Health is operated under the authority of a Joint Powers Board with representation from each member county. PrimeWest contracts with the state (DHS) and the federal government (CMS) to provide services to people enrolled through one of the state's public health plans as well as those enrolled through Medicare.

South Country Health Alliance (SCHA) is operated under the authority of a Joint Powers Board with representation from each of the member counties. SCHA contracts with the state (DHS) and the federal government (CMS) to provide services to people enrolled through one of the state's public health plans as well as those enrolled through Medicare.
