Find Certified Health Care Homes

Choose one of the following:
The map may be blank if there are not data with those filter combinations.

1. Select Clinic Certification Level: All clinic certification levels (Foundational Level, Level 2, and Level 3) are initially displayed. To select one particular level, click in the Clinic Certification Level area and choose the desired level.
   Click in the white space to clear the search.
2. Select Clinic County: All counties are initially displayed. To select a specific county, uncheck (All) and use the slider to scroll down to find the desired county.
   Click (All) at the top of the list to clear the search.
3. Select Organization: All organizations are initially displayed. To select a specific organization, uncheck (All) and use the slider to scroll down to find the desired organization.
   Click (All) at the top of the list to clear the search.

For keyboard only users, please refer to the data available in the excel file above.

When interacting with the map, hover the cursor over a clinic to see a pop-up showing:

• Organization Name
• Clinic Name
• Clinic Certification Level
• Clinic Street
• Clinic City, State, and Postal Code.

When interacting with the map, navigation options will appear in the upper left corner:

• Increase the map size, click the “+” sign
• Decrease the map size, click the “-“ sign
• To see different areas of the map, left click and drag
