Employee Illness Log

The person-in-charge is required to notify the local health department or MDH if any food employees are known to be infected with Salmonella, Shigella, E. coli, the hepatitis A virus, or other pathogen that can be transmitted through food. The person-in-charge is also required to record all reports of diarrhea or vomiting in food employees and have these records available to the licensing health department upon request. This log can be used to track and report employee illnesses.
