MDH tracks outbreaks of acute respiratory illness in Minnesota K-12 schools during the school year. Long-term care facilities (nursing homes and assisted living facilities) report when an outbreak of influenza or RSV has occurred as well as the number of cases of COVID-19 in their facility. 
Note: The most recent weeks on the graphs are marked as “data pending” as we know information is not complete. 

  • All data are preliminary and may change as information is received. Data trends are based on statewide rates.

Resident Cases Associated with Assisted Living Facilities

  • Download: Long-term/congregate care data (CSV)
    Cases of COVID-19 in residents and staff associated with assisted living and skilled nursing facilities are by specimen collection date.
    • As of 10/31/2024 staff cases are no longer being reported. Historical data is included in the downloadable CSV file.
    • As of 7/3/25 weekly resident COVID-19 case counts in skilled nursing facilities are no longer available. Nursing home cases were updated as of 3/25/25 when the most recent NHSN facility-level data were published. Historical data is included in the downloadable CSV file. Please see the link below to the NHSN Nursing Home Data Dashboard for weekly case rates among nursing home residents, which can be filtered to Minnesota for statewide numbers.

    The NHSN Nursing Homes Data Dashboard includes some COVID-19 Minnesota nursing home case rates.
  • CMS: COVID-19 Nursing Home Data
    Information on COVID-19 reported by nursing homes to the CDC’s National Healthcare Safety Network (NHSN) COVID-19 Long Term Care Facility Module.
  • Deaths among cases that resided in long-term care or assisted living facilities (cumulative): 8,363
