About the SagePlus Heart Health Assessment Program
SagePlus is looking for partner clinics to implement free heart health services for underinsured and uninsured women. This work aims to improve the health among communities that experience the greatest disparities. Funding supports implementation of heart health screening and follow-up, social services needs assessments and referrals, and lifestyle change programming (e.g., exercise or nutrition classes).

About SagePlus
SagePlus is a heart health and lifestyle change program for individuals eligible for the Sage Breast and Cervical Cancer Screening Program. SagePlus is a heart health and lifestyle change program for individuals who are eligible for the Sage Breast and Cervical Cancer Screening Program.
Program components include heart health assessments, education on risk factors, and ongoing support for lifestyle changes as well as addressing barriers participants experience (e.g., access to transportation to medical appointments, food, housing, etc.).
Services are provided at participating clinics and community locations free of charge.
Eligibility
- Women (born female or transgender women and men) ages 35-64
- Uninsured and underinsured
- Income within program guidelines

Services
- Assessment of heart and stroke risk factors, including clinical labs, eating and physical activity habits, and others.
- Patient education on risk factor results
- Follow-up tests for patients with abnormal screening results
- Ongoing support for healthy behavior lifestyle changes
- Social services and supports for barriers to health such as transportation to medical appointments, access to food, housing, etc.
- Follow-up assessment to show results of lifestyle change activities.

The SagePlus model
Healthy behavior support services
Heart disease and other chronic conditions are tied closely to where people live, work, play, and pray. Factors such as being able to find and afford nutritious food, experiencing discrimination and violence, or having safe housing and transportation and a trusted support system all contribute to health. These factors that impact health are known as social determinants of health.
SagePlus offers healthy behavior support services (HBSS) to improve lifestyle habits using an evidence-informed, team-based care model. The SagePlus program uses a cultural lens and incorporates health coaching to improve engagement and help participants complete the program.
The program also helps connect patients to food, transportation to medical appointments, and other resources to address barriers to health. Additionally, rewards like grocery gift cards encourage participants to attend the program and make healthy lifestyle changes.
Format
The SagePlus program is offered in two different HBSS formats: Clinics options and community options.
Clinic options
There are three reimbursable options for clinic participation:
- Nutrition education
- Diabetes education
- Health and wellness coaching
Community options
Listed below are community-based, culturally-relevant programs:
- Le Me Se Se: a simple cardiac lifestyle change program designed specifically for women of African descent. Le Me Se Se means "good health" in the Ewe language.
- Walk with Ease
- Yoga
- Zumba
Billing and reimbursement
