Learning Days Event

The annual Learning Days conference brings together more than 200 health care providers, public health professionals and community partners to exchange information, advance health equity, and build relationships.

The 2026 Learning Days conference will be held Wednesday May 13 at the Heritage Center of Brooklyn Center.

Please join us for this annual spring event where Health Care Homes, public health, and community partners come together to share best practices, learn from each other, and form collaborative relationships.

The Health Care Homes Program is now accepting presentation proposal submissions for Health Care Homes Learning Days 2026, to be held on Wednesday, May 13, 2026, at the Heritage Center of Brooklyn Center.

Health Care Homes Learning Days is a statewide conference designed to support learning and collaboration among healthcare professionals, community partners, and organizations working to improve patient and population health. The conference focuses on practical strategies that strengthen patient and family engagement, care coordination, integrated care teams, and equitable approaches to care delivery.

We are seeking proposals for one hour breakout sessions that share effective practices, innovative tools, and real-world experiences that participants can apply in their work.

The deadline to submit a presentation proposal is January 30, 2026.

Health Care Homes Learning Days is an annual, in-person conference hosted by the Minnesota Department of Health. The conference brings together primary care teams, clinic leaders, community partners, and system stakeholders to share practical strategies that strengthen care coordination, advance health equity, and support integrated, patient-centered care across Minnesota.

Standard Exhibitor – $300

Sponsorship Opportunities

Sponsorship opportunities provide targeted visibility and meaningful engagement while supporting access, equity, and the long-term sustainability of Health Care Homes Learning Days. All sponsors will be recognized in the conference app and may provide a logo, brief description, and link to their organization’s website for attendee access. Each sponsoring organization will receive a dedicated link to submit this information for inclusion in the app.

Premier Conference Sponsor – $2,000

  • Highest level of conference recognition
  • Logo recognition on the Health Care Homes Learning Days webpage
  • Logo displayed on opening and closing conference slides
  • Verbal recognition during opening and closing remarks
  • Two complimentary conference registrations
