Critical Access Hospital (CAH) Program Information
The Critical Assess Hospital designation is given to eligible rural hospitals by the Centers for Medicare and Medicaid Services (CMS).

Closed Hospitals (Excel)
This list contains addresses and the date closed. Some designated hospitals are only closed for the purposes of independent data reporting under statutory requirements. The facility is open but operating as an offsite hospital campus of another licensed hospital. All financial, utilization, and services data is included in the licensed hospital's information.
