Continuous Quality Improvement (CQI)
The Minnesota Department of Health (MDH) family home visiting section is dedicated to supporting continuous quality improvement (CQI) efforts at both the state and local levels to strengthen the delivery of home visiting services for families with young children. CQI is an ongoing process of using data, feedback, and small tests of change to regularly improve practices and outcomes.

Monthly CQI connect and learn
MDH hosts a monthly CQI Connect and Learn for Strong Foundations grantees. Come join the conversation with MDH and family home visiting programs across the state to brainstorm with peers, discuss performance measures and program outcomes, and learn more about CQI. Meetings are held via Microsoft Teams and alternate between day and time depending on the month. Registration is required. Click on the links below to register for each of the upcoming dates.

CQI projects
MDH supports family home visiting programs across Minnesota with different projects and collaboratives. MDH uses the Institute of Healthcare Improvement’s Breakthrough Series collaborative model as a framework for CQI in family home visiting.
CQI focus areas are typically selected on an annual basis, often guided by input from internal and external stakeholders, issues of high interest at both the state and local program levels, and areas identified as needing improvement based on national family home visiting performance benchmarks. Below are the topic areas and related documents for the current and recent statewide CQI learning collaboratives.

In 2024, MDH supported a CQI project related to family retention, with a focus on increasing prenatal enrollment. Efforts were made to decrease missing data in MIECHV performance measures.

In 2023, MDH is supported a CQI project related to family engagement and family retention. Two local family home visiting agencies also participated in the CQI Practicum with MDH. The 2023 CQI Practicum Showcase highlights this project.
MDH also created a poster to showcase qualitative data that was collected during 2023. This poster, A 2023 MN Initiative: Collaborative Use of Qualitative Data in Continuous Quality Improvement Efforts (PDF), was presented at the MIECHV All Grantee Meeting in January 2024.
