Manage Your Diabetes for Life Toolkit
Diabetes resource to help guide people with diabetes
People with diabetes are encouraged to receive diabetes education from an experienced care team at initial diagnosis and at critical points during the condition. Enrolling in an accredited or recognized Diabetes Self-Management Education and Support (DSMES) Program is highly recommended.
The Manage Your Diabetes For Life Toolkit provides additional education and support to people with diabetes. The material is designed for anyone, health care providers, health educators, case managers, health coaches, community health workers, interpreters, patient advocates, caretakers, and family members, to guide someone with diabetes through the material.
The toolkit information is also useful to help people with pre-existing diabetes manage their diabetes and enjoy life.
