Stroke Program Funding Opportunities

Stroke Team-Based Care: Request for Proposals

The Minnesota Department of Health (MDH) Stroke Program is requesting proposals from qualified organizations for innovative solutions to enhance the use of team-based care to improve communication, coordination, and collaboration in the care of stroke survivors and individuals at risk for stroke.

Important dates

  • Last day to submit RFP questions: Monday, October 6, 2025
  • Applications due: Wednesday, October 15, 2025
  • Estimated date applicants to receive award and denial notice: November 2025
  • Grant Start Date: Friday, January 2, 2026

Funding

MDH estimates two grantees will be awarded $15,000 each for one (1) year. This grant opportunity is a one-year grant with the possibility to extend two years based on funding availability.

Application Submission

Applications must be submitted into REDcap Stroke Team-Based Care Application by Wednesday, October 15, 2025.

Question and Answers

All questions regarding this RFP must be submitted by email to health.stroke@state.mn.us. All questions and answers will be posted on this page within three business days. 

Please submit questions no later than 4:30 p.m. Central Time on Monday, October 6, 2025.


Question: If our county is not listed under the priority geographies but has a hypertension prevalence of 31% (based on MN EHR Consortium Data) and borders a county that is listed under the priority geographies, and has the same EMS service covering both counties, are we still eligible to apply?

Answer: Yes, you can consider applying if your county serves people from a neighboring county that is listed under the priority geography list. In addition to serving people in priority geographies, please describe the needs of those people to meet the criteria for either of the two priority populations below.

  • Working-age (aged 18-64) stroke patients and those at risk for stroke
  • Stroke survivors with unmet rehabilitation, health, and support needs

Question: Is it alright for there to be two applications from the same organization?

Answer: Yes, it is possible for more than one application to come from the same organization, as long as each proposal is distinct and meets eligibility requirements outlined in the RFP.

Question: Is the grant funding opportunity stroke program specific or system?

Answer: The grant is not limited to a single stroke program or system. We're looking for proposals from settings across the stroke continuum of care that strengthen coordinated, team-based care for stroke patients or those at high risk for stroke.
