Home Care License and Registration Renewal
License and Registration Renewal
Home care licenses and home management registrations are renewed annually. Licensees/registrants will receive renewal information approximately 60-90 days prior to the expiration date of their license/registration to the email on file with the department. If you do not receive a renewal letter 30 days prior to your license expiration date, please contact Health.Homecare@state.mn.us.
Home care licensees with the HCBS designation will renew the designation at the same time as the home care license renewal. If you wish to drop the HCBS designation, you must notify MDH in writing prior to completing your home care license renewal.

Home Care & Integrated License Renewal Eligibility
Home Care License
Under Minnesota Statutes, section 144A.472, subdivision 3 (3), a home care provider must have delivered qualifying home care services within the past 12 months, for a fee, to be eligible for license renewal.
If a provider has not delivered at least one billable home care service to a client in their home during the past 12 months, the provider is not eligible to renew and will be asked to voluntarily close the license.
Integrated License (Home Care + 245D Basic Support)
If you hold an integrated license, the same renewal eligibility requirements apply under Minnesota Statutes, section 144A.472, subdivision 3 (3), in addition to Minnesota Statutes, section 144A.484, subdivision 4. The provider must deliver at least one billable home care service, for a fee, to each client who is receiving 245D basic support services.
If home care services are not provided to each client receiving 245D basic supportive services, the provider is not eligible to renew and will be asked to voluntarily close the integrated (245D) HCBS designation.
To verify renewal eligibility, and in accordance with Minnesota Statutes, section 144A.472, subdivision 3 (3) and (5), MDH requires licensees to submit supporting documentation, including a client service agreement and a billing statement.
Home Care Licenses
Home care licenses are renewed online. Your renewal letter will have the login information you need to complete the online renewal.
Temporary Licensed Home Care Providers
Temporary licensed home care providers must pass an initial full survey before they can receive a license. Temporary licensees must inform the Department of Health within five days of beginning to provide home care services.
While some providers hold an integrated license that allows delivery of 245D basic support services, these services cannot substitute for the statutory requirement to provide a home care service under 144A. While both HCBS and home care may involve assisting with activities of daily living (ADLs), they are distinct for purposes of licensure and payment. Providing HCBS basic support services alone does not meet the definition of a home care provider and does not qualify the agency for survey eligibility.
Once MDH receives notification that you are providing home care services, we will conduct an unannounced on-site survey. After the survey you will receive further information about any compliance issues and your license status. If the license is granted, you will receive renewal instructions.
Home Management Registrations
Home management registrations are renewed via paper application. We will mail you the application before your expiration date. Complete the application and return it with your application fee and supporting documentation, as required.
